Primary Care Physician (MD/DO) Overview
Our team is seeking a dedicated Primary Care Physician (MD/DO) to join a physician-led, mission-driven primary care organization in Jacksonville, FL. This full-time role offers a unique opportunity to be part of a healthcare provider committed to transforming the care of seniors and individuals with complex health needs. As a Primary Care Physician (MD/DO), you will be empowered to deliver high-quality, patient-centered, value-based care, supported by a collaborative care team in an outpatient clinic setting.
Responsibilities
- Provide comprehensive primary care services to a dedicated panel of adult and senior patients, focusing on those with multiple chronic conditions.
- Manage a patient panel with a maximum of 450 patients, allowing for personalized attention and continuity of care.
- Conduct thorough clinical assessments, develop individualized care plans, and manage ongoing treatment for complex medical conditions.
- Work collaboratively with a multidisciplinary support team, including a medical assistant, scheduler, and referral specialist, to optimize patient outcomes and experience.
- Participate in value-based care initiatives and implement evidence-based clinical protocols to enhance patient health and satisfaction.
- Maintain a Monday through Friday schedule, with clinic hours from 7:30 a.m. to 5:00 p.m. (last patient scheduled at 4:15 p.m.), ensuring work-life balance with no required nights or weekends.
- Utilize allocated administrative time within your schedule for documentation, care coordination, and professional development.
- Engage with in-house consulting specialists, including cardiology and podiatry, and leverage holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling offered by the clinic.
- Support patient access by coordinating with transportation services provided at no charge to eligible patients.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree required.
- Completion of an accredited residency program in Internal Medicine (IM) or Family Medicine (FM) is required.
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Strong commitment to value-based, patient-centered care, with a passion for serving senior and medically complex populations.
- Excellent interpersonal, communication, and organizational skills.
