The Primary Care Physician (MD/DO) role is structured to support work-life balance. You will enjoy a Monday through Friday schedule with regular clinic hours and no night or weekend shifts. Administrative time is built into your schedule, ensuring you have the opportunity to manage patient care efficiently and effectively. The practice is 100% outpatient, serving a maximum panel of 450 patients, primarily seniors with multiple chronic conditions. You will also benefit from the support of a dedicated Medical Assistant, Scheduler, and Referral Specialist, enabling you to focus on clinical care and patient relationships.
Our clinics offer a wide array of in-house consulting specialists, including Cardiology and Podiatry, to support comprehensive patient care. Holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling are also available, reflecting our commitment to a whole-person approach to wellness. For patients requiring assistance, door-to-door transportation is provided at no charge, further enhancing access to care.

Primary Care Physician (MD/DO) - Summary- Provide comprehensive, patient-centered primary care to a senior patient population in an outpatient setting.
- Collaborate with a multidisciplinary care team including Medical Assistants, Schedulers, and Referral Specialists.
- Leverage value-based care principles to optimize patient outcomes and preventative health measures.
- Deliver high-quality primary care to a panel of up to 450 patients, focusing on seniors with multiple chronic diseases.
- Conduct patient visits, manage chronic conditions, and provide preventative care.
- Utilize administrative time within your schedule for patient documentation, care coordination, and follow-up.
- Work collaboratively with in-house consulting specialists and leverage holistic health services offered by the clinic.
- Participate in value-based care delivery, measuring and striving for better health outcomes.
- Engage in ongoing professional development and contribute to the practice's mission and vision.

- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
