Primary Care Physician (MD/DO) Job OverviewWe are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led, mission-driven organization. This full-time opportunity is based in Jacksonville, FL, and is ideal for those passionate about providing comprehensive care to seniors and medically complex patients. Responsibilities- Provide comprehensive primary care for a dedicated panel of up to 450 patients, focusing on seniors with chronic health conditions.
- Deliver value-based care in a fully outpatient setting, supporting improved outcomes and patient satisfaction.
- Conduct patient visits Monday through Friday during clinic hours of 7:30 a.m. to 5:00 p.m., with the last patient scheduled at 4:15 p.m.
- Manage an average of 18 patients per day, allowing for high-quality, personalized care and work/life balance.
- Collaborate with a dedicated care team including a Medical Assistant, Scheduler, and Referral Specialist to optimize patient experience and care coordination.
- Participate in value-based care initiatives focused on delivering better health outcomes and honoring the needs of senior patients.
- Utilize administrative time included in your schedule to complete documentation and care planning.
- Engage with in-house consulting specialists such as Cardiology and Podiatry, and leverage holistic health services when appropriate.
- Ensure compliance with organizational policies, professional standards, and ethical guidelines.
- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Demonstrated commitment to providing high-quality, compassionate care to seniors and patients with chronic conditions.
- Strong interpersonal and communication skills, with an ability to work effectively within a multidisciplinary team.
