Primary Care Physician (MD/DO) Overview
Our organization is seeking a dedicated Primary Care Physician (MD/DO) for a full-time outpatient role in Jacksonville, FL. Responsibilities
- Provide comprehensive primary care services to a small patient panel, with a focus on seniors managing multiple chronic conditions.
- Manage a daily schedule averaging 18 patients, ensuring time for both clinical care and administrative tasks within regular clinic hours (Monday – Friday, 7:30 am – 5:00 pm).
- Deliver 100% outpatient care with no night or weekend shifts, supporting a healthy work/life balance.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to ensure coordinated and efficient patient care.
- Utilize a value-based care approach to improve patient outcomes and support organizational goals.
- Participate in partnership opportunities, with most physicians achieving partner status within 12-18 months.
- Engage in ongoing professional development and contribute to a culture of excellence within the clinic.
- Leverage in-house consulting specialists and holistic health services to offer comprehensive care options to patients.
- Support patient access through services such as door-to-door transportation for those in need.
Qualifications
- Medical degree (MD or DO) with completion of a residency in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated experience or strong interest in managing care for seniors with multiple chronic diseases.
- Commitment to value-based care and continuous improvement in patient outcomes.
- Excellent communication, teamwork, and organizational skills.
