Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our mission-driven primary care organization in Tampa, FL. This is a full-time opportunity to deliver high-quality outpatient care to a senior patient population, with a focus on value-based healthcare. Responsibilities
- Manage a panel of up to 450 patients, primarily seniors with multiple chronic conditions, providing comprehensive and coordinated care.
- Conduct outpatient visits for up to 18 patients per day, ensuring thorough assessments and individualized treatment plans.
- Work collaboratively with a dedicated support team, including a medical assistant, scheduler, and referral specialist, to enhance patient care and clinic operations.
- Engage in value-based care initiatives to improve patient outcomes and support the organization's mission of transformative healthcare.
- Utilize administrative time built into the schedule for documentation, care coordination, and follow-up tasks.
- Participate in a Monday through Friday clinic schedule, with clinic hours from 7:30 am to 5:00 pm, ensuring work-life balance. No nights or weekends are required.
- Refer patients to in-house consulting specialists such as cardiology and podiatry as needed, and collaborate on holistic health services including acupuncture, Tai Chi, yoga, and nutrition counseling.
- Maintain compliance with all clinical protocols and organizational policies, supporting continuous quality improvement efforts.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Strong commitment to providing patient-centered care in an outpatient, value-based care setting.
- Excellent communication and teamwork skills, with the ability to work effectively with multidisciplinary teams.
