Primary Care Physician (MD/DO) - Summary
The Primary Care Physician (MD/DO) serves as the cornerstone of our outpatient clinic, managing a small, focused panel of senior patients with multiple chronic conditions. You will work closely with a multidisciplinary team, including medical assistants, schedulers, and referral specialists, to deliver patient-centered, value-based care. Administrative support and in-house consulting specialists are available to streamline your workflow, allowing you to dedicate more time to patient care and less to paperwork.
Duties & Responsibilities
- Provide comprehensive primary care services to a panel of up to 450 senior patients.
- Conduct outpatient visits for an average of 18 patients per day, focusing on seniors with 3-5 chronic conditions.
- Participate in value-based care initiatives to improve patient outcomes and satisfaction.
- Collaborate with a dedicated care team, including an MA, scheduler, and referral specialist.
- Utilize in-house consulting specialists for cardiology, podiatry, and other services as needed.
- Incorporate holistic health services such as acupuncture, tai chi, yoga, and nutrition into patient care plans.
- Maintain thorough documentation and ensure compliance with all regulatory requirements.
- Engage in administrative activities during scheduled time blocks within the workday.
- Foster a supportive and inclusive environment that aligns with the organization’s mission and values.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Completed residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to delivering high-quality, patient-centered care for seniors.
- Excellent communication and collaboration skills.
- Demonstrated ability to work effectively within a multidisciplinary care team.
- Interest in value-based, preventive, and holistic care models.
