Primary Care Physician (MD/DO) - Summary
- Deliver value-based, outpatient primary care in a supportive, team-oriented environment.
- Focus on adult and senior populations with complex, chronic health needs.
- Work with a manageable patient panel size to maximize quality of care.

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 patients, focusing on seniors with multiple chronic conditions.
- Conduct routine wellness visits, preventive screenings, and chronic disease management.
- Collaborate with a multidisciplinary care team, including medical assistants, schedulers, and referral specialists.
- Utilize value-based care practices to improve patient outcomes and experience.
- Participate in administrative and care coordination activities within the clinical schedule.
- Work collaboratively with in-house consulting specialists, including cardiology and podiatry, to coordinate holistic health plans.
- Leverage holistic health services offered by the clinic, such as acupuncture, tai chi, yoga, and nutrition counseling, to support patient wellness.
- Maintain accurate and timely documentation in accordance with organizational and regulatory standards.
- Engage in ongoing professional development and contribute to clinic quality improvement initiatives.
- Schedule: Monday through Friday, with clinic hours from 7:30 am to 5:00 pm; last patient scheduled at 4:15 pm. No nights or weekends required.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Demonstrated commitment to value-based, patient-centered care.
- Strong interpersonal, organizational, and communication skills.
- Ability to work collaboratively in a multidisciplinary team environment.
