Primary Care Physician (MD/DO) – Summary
- Full-time, outpatient-based role delivering value-based care to seniors.
- Serve a patient panel of approximately 450 individuals, allowing for in-depth, personalized care.
- Focus on managing 3-5 chronic diseases per patient within a holistic care model.
- Work closely with a dedicated support team and consulting specialists.
Duties & Responsibilities
- Provide comprehensive primary care services to senior patients in an outpatient clinic setting.
- Manage a daily schedule of approximately 18 patients, with administrative time included.
- Collaborate with a multidisciplinary care team, including medical assistants, schedulers, and referral specialists.
- Utilize value-based care practices to improve patient health outcomes and efficiency.
- Engage in holistic health initiatives, such as coordinating with in-house specialists and supporting lifestyle interventions like acupuncture, Tai Chi, yoga, and nutrition counseling.
- Maintain accurate and timely documentation of patient encounters.
- Participate in partnership opportunities, with the potential for advancement within 12-18 months.
- Ensure compliance with organizational protocols and quality standards.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
