Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our mission-driven organization, committed to providing high-quality, value-based healthcare to seniors. This is a full-time opportunity situated in New Orleans, LA, where you will be part of a team focused on delivering comprehensive primary care services to a patient population with complex needs. Responsibilities
- Provide comprehensive, patient-centered primary care to a panel of up to 450 senior patients, primarily with multiple chronic conditions.
- Deliver care in a 100% outpatient setting, supported by a dedicated care team including a medical assistant, scheduler, and referral specialist.
- Conduct thorough assessments, develop individualized care plans, and manage chronic conditions utilizing a value-based care model.
- See an average of 18 patients per day, allowing for quality time and relationship-building with each patient.
- Participate in in-house collaborative care with consulting specialists in areas such as cardiology and podiatry, as well as holistic health services like acupuncture, Tai Chi, yoga, and nutrition counseling.
- Maintain a regular Monday through Friday schedule, with clinic hours from 7:30 am to 5:00 pm (last patient scheduled at 4:15 pm).
- Ensure administrative duties are completed during allotted administrative time within your daily schedule.
- Engage in partnership opportunities with no buy-in required, typically available within 12-18 months, with a high rate of providers achieving partnership within the first year.
- Support patient access to care, with transportation provided at no cost to those in need.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to delivering value-based, patient-centered care to senior populations.
- Excellent interpersonal, communication, and organizational skills.
