In this full-time Primary Care Physician (MD/DO) role, you will work within a collaborative and supportive environment, allowing you to focus on delivering high-quality, relationship-driven care. Working Monday through Friday, you’ll experience excellent work/life balance, with no nights or weekends required. Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Provide comprehensive, patient-centered primary care to a maximum panel of 450 patients, focusing on seniors with multiple chronic conditions.
- Conduct thorough assessments, develop individualized care plans, and coordinate treatment for a diverse geriatric population.
- Work collaboratively with a multidisciplinary clinical team, including in-house specialists (e.g., cardiology, podiatry).
- Utilize holistic health services such as acupuncture, tai chi, yoga, and nutrition as part of patient care plans when appropriate.
- Manage daily patient flow, typically seeing 18 patients per day, with administrative time built into your schedule.
- Ensure clear documentation, follow-up, and communication with patients and families.
- Champion value-based care initiatives, focusing on quality metrics, preventive care, and cost-effective outcomes.
- Participate in ongoing professional development and clinic improvement efforts.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board certified or eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to providing compassionate, high-quality care to seniors and medically complex patients.
- Ability to work effectively in a team-based, value-driven clinical setting.
- Excellent interpersonal, communication, and organizational skills.
