Primary Care Physician (MD/DO) – Summary
- Full-time opportunity for a Primary Care Physician (MD/DO)
- Focus on senior patient population with complex, chronic conditions

Duties & Responsibilities
- Provide comprehensive outpatient primary care to a panel of up to 450 patients, primarily seniors
- Manage an average of 18 patients per day in a value-based care setting
- Develop and implement individualized care plans for patients with 3-5 chronic diseases
- Collaborate with dedicated care teams, including medical assistants, schedulers, and referral specialists
- Participate in regular administrative time built into your schedule
- Engage in partnership opportunities, typically available within 12-18 months, with the majority achieving partner status after one year
- Work a Monday through Friday schedule with clinic hours from 7:30 am to 5:00 pm (last patient scheduled at 4:15 pm)
- Ensure continuity of care using a holistic and patient-centered approach
- Refer and coordinate care with in-house specialists such as cardiology and podiatry
- Promote health and wellness through access to holistic services including acupuncture, tai chi, yoga, and nutrition counseling
- Support organizational mission and contribute to a culture of clinical excellence and patient advocacy

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board certified or board eligible in FM, IM, or Geriatric Medicine
- Commitment to value-based, patient-centered care for seniors
- Excellent communication, collaboration, and organizational skills
- Demonstrated ability to work effectively within a multidisciplinary team
- Current, unrestricted medical license
