Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to join our mission-driven, physician-led organization in Chicago, IL. This is a full-time outpatient opportunity to make a significant impact on the health and well-being of seniors in the Chicago community. As a Primary Care Physician, you will work within a collaborative team environment, focusing on value-based care and providing comprehensive services to a population with complex chronic needs. Key Responsibilities
- Deliver primary care to a panel of up to 450 senior patients, primarily managing those with three to five chronic diseases.
- Provide care in a 100% outpatient setting, collaborating with a dedicated support team including a Medical Assistant, Scheduler, and Referral Specialist.
- Conduct patient visits during clinic hours, Monday through Friday, from 7:30am to 5:00pm, with the last patient scheduled at 4:15pm. No nights or weekends required, promoting an excellent work/life balance.
- See up to 18 patients daily, ensuring sufficient time for quality care and administrative responsibilities.
- Utilize value-based care models to deliver effective, patient-centered care that improves health outcomes and patient satisfaction.
- Engage with in-house consulting specialists such as cardiology and podiatry, and refer patients as needed to holistic health services including acupuncture, tai chi, yoga, and nutrition counseling.
- Coordinate care with other healthcare professionals and facilitate patient access to door-to-door transportation services.
Qualifications
- MD or DO degree from an accredited medical school.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to providing high-quality, compassionate primary care to a diverse senior patient population.
- Strong collaborative skills and the ability to work effectively as part of a multidisciplinary team.
- A passion for value-based care and improving the health of underserved communities in Chicago, IL.
