In this full-time Primary Care Physician (MD/DO) role, you will deliver comprehensive outpatient care to a senior patient population, typically managing individuals with multiple chronic illnesses such as heart disease, diabetes, and hypertension. Our care model emphasizes preventative health, holistic wellness, and the reduction of avoidable hospitalizations. You will be supported by a dedicated care team, including medical assistants, schedulers, referral specialists, and responsive leadership, ensuring you can focus on providing the highest quality of care.
Our organization also goes beyond traditional healthcare by offering complimentary transportation options for patients who do not drive, such as Uber and Lyft, and providing no-cost wellness activities including exercise and art classes. The Primary Care Physician (MD/DO) will work in a collaborative, team-based atmosphere, with a manageable daily patient load of 10-12, allowing for thorough and personalized care. A maximum panel size of 650 patients ensures focus on quality over quantity. The position follows a standard Monday through Friday, 8am-5pm schedule, with call rotation dependent on the number of providers in the market (for example, a 1:32 week call schedule if there are 32 physicians).

Duties & Responsibilities
- Provide comprehensive primary care services to adult and senior patients, with a focus on chronic disease management and preventative care.
- Conduct thorough patient assessments, develop individualized care plans, and manage ongoing treatment for conditions such as cardiovascular disease, diabetes, and hypertension.
- Perform in-office procedures such as suturing, joint injections, skin biopsies, and basic wound care, as appropriate for scope of practice.
- Collaborate with a multidisciplinary care team, including medical assistants, schedulers, and referral specialists, to coordinate seamless patient care.
- Utilize electronic health records for accurate and timely documentation.
- Engage patients in wellness activities and encourage participation in community-based health programs offered at the clinic.
- Participate in a shared call rotation based on the number of providers in the region.
- Adhere to organizational policies, clinical guidelines, and value-based care metrics such as patient satisfaction, HEDIS compliance, and reduction of hospitalizations.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certification or Board Eligibility in Family Medicine (FM) or Internal Medicine (IM) is required.
- Active, unrestricted medical license to practice medicine.
- Current DEA registration.
- Proficiency in common office-based procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in geriatrics is a plus, but not required.
- Excellent interpersonal, communication, and organizational skills.
