Primary Care Physician (MD/DO) - Summary
- Full-time outpatient primary care role focusing on senior patients with multiple chronic conditions
- Value-based care model emphasizing continuity, prevention, and patient satisfaction
- Work collaboratively with a dedicated care team and supportive leadership
- Clinic-based position in Kansas City, MO with a start date in May 2025
Duties & Responsibilities
- Provide comprehensive primary care to a panel of senior patients (max panel size of 650)
- Deliver care for patients with 3-5 chronic conditions, including heart disease, diabetes, and hypertension
- Manage an average daily patient load of 10-12 to prioritize quality, personalized care
- Coordinate with your own support team, including medical assistants, schedulers, and referral specialists
- Participate in value-based care initiatives, focusing on HEDIS compliance, reducing hospital admissions, and enhancing patient satisfaction
- Maintain high standards of clinical documentation and communication
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and wound care as needed
- Participate in periodic call schedules (e.g., 1:32 weeks based on provider count)
- Engage in ongoing professional development and quality improvement activities

Qualifications & Requirements
- Medical degree (MD or DO) required
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Active, unrestricted license to practice medicine
- Current DEA registration
- Proficiency in basic office procedures: suturing, joint injections, skin biopsies, and wound care
- Fellowship in Geriatrics is a plus but not required
- Demonstrated commitment to value-based care and patient-centered medicine
