Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative healthcare team in Oklahoma City, OK. This full-time, outpatient opportunity offers an engaging, patient-centered environment where you can make a significant impact on the lives of seniors with complex health needs. As a Primary Care Physician, you will play a vital role in fostering a wellness-focused community and ensuring patients receive personalized, respectful care.
Responsibilities
- Provide comprehensive primary care to a panel of senior patients, focusing on those with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Deliver care in a state-of-the-art, outpatient clinic operating Monday through Friday, 8:00 a.m. to 5:00 p.m.
- Manage a patient panel of up to 650 individuals, averaging 10-12 patient visits per day to ensure thorough, quality interactions.
- Utilize a value-based care model to optimize patient outcomes, emphasizing preventive care, chronic disease management, and reducing unnecessary hospitalizations.
- Work collaboratively with a dedicated care team, including medical assistants, schedulers, referral specialists, and robust leadership support.
- Participate in a rotating call schedule, determined by the total number of physicians in the market (e.g., 1:32 weeks if 32 providers).
- Engage patients and their families in health education, wellness activities, and community-based programs offered by the clinic, such as transportation support and senior enrichment activities.
- Maintain compliance with HEDIS measures, support patient satisfaction initiatives, and demonstrate the organization’s core values in daily practice.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Active, unrestricted medical license to practice medicine in Oklahoma.
- Current DEA registration.
- Proficient in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is a plus, but not required.
