As a Primary Care Physician (MD/DO), you will work in a collaborative and supportive outpatient clinic, providing comprehensive care to a dedicated panel of senior patients. Primary Care Physician (MD/DO) – Summary
- Full-time outpatient role serving a panel of senior patients with complex, chronic health needs
- Mission-driven, physician-led organization focused on value-based, patient-centered care

Duties & Responsibilities
- Provide high-quality primary care to a panel of up to 450 senior patients, averaging 18 patients per day
- Conduct comprehensive assessments, develop care plans, and manage chronic disease in a value-based care setting
- Utilize a multidisciplinary team including medical assistant, scheduler, and referral specialist for optimal patient support
- Work closely with in-house specialists such as cardiology and podiatry, and refer as needed
- Incorporate holistic health services (acupuncture, tai chi, yoga, nutrition) into patient care as appropriate
- Schedule includes Monday to Friday clinic hours, with no nights or weekends required
- Dedicate administrative time within your regular schedule; last patient scheduled at 4:15 PM
- Participate in partnership opportunities and clinical leadership initiatives
- Promote a patient-centered, respectful, and inclusive care environment
Salary & Benefits
- Competitive base salary starting at $250,000, plus incentive bonus potential
- Signing bonus of up to $50,000
- Annual bonus structure, with additional incentives for partners
- Comprehensive benefits package including health, dental, vision, and supplemental plans for providers and families
- 401(k) retirement plan with 5.5% employer match
- Paid time off totaling 33 days per year
- CME allowance of $3,500 plus one week off
- Door-to-door transportation for patients provided at no charge
Qualifications & Requirements
- Residency trained in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board certified or board eligible in IM, FM, or Geriatric Medicine
- Commitment to value-based, patient-centered primary care
- Strong communication, organizational, and collaboration skills
- Dedication to serving older adults with multiple chronic conditions
