Primary Care Physician (MD/DO) – Summary

Duties & Responsibilities
- Provide comprehensive outpatient primary care to a panel of up to 650 patients, with a typical daily caseload of 10-12 seniors managing 3-5 chronic conditions such as heart disease, diabetes, and hypertension
- Utilize a value-based care approach focused on preventive health, disease management, and patient satisfaction
- Coordinate care with a dedicated team including a medical assistant, scheduler, referral specialist, and leadership support
- Participate in performance improvement initiatives, ensuring adherence to HEDIS compliance and company values
- Engage in regular communication with patients and families, promoting education, wellness, and continuity of care
- Maintain accurate and timely medical records in compliance with organizational and regulatory standards
- Perform in-office procedures (suturing, joint injections, skin biopsies, wound care) as required
- Participate in call schedule as determined by the number of providers in the region
- Promote and participate in wellness activities offered at the clinic

- Competitive base salary with total earning potential exceeding $300,000, including performance-based bonuses

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Board certified or board eligible in Family Medicine (FM) or Internal Medicine (IM)
- Current, unrestricted, and active license to practice medicine
- Current DEA registration
- Proficient in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Fellowship training in Geriatrics is a plus but not required
- Strong commitment to value-based, patient-centered care delivery
