Primary Care Physician (MD/DO) About the Role
This full-time Primary Care Physician (MD/DO) position is based in Phoenix, AZ, with an innovative healthcare provider dedicated to delivering exceptional patient care in community-based outpatient clinics. As a Primary Care Physician, you will play a vital role in improving the health outcomes of patients, focusing primarily on a senior population with complex medical conditions. The organization is committed to a value-based care model, emphasizing patient satisfaction, continuity, and wellness-driven programming in a supportive environment.
Key Responsibilities
- Provide comprehensive outpatient primary care to adult and senior patients, focusing on those with multiple chronic conditions such as heart disease, diabetes, and hypertension.
- Work within a value-based care framework to ensure high-quality, patient-centered outcomes.
- Maintain a panel size of up to 650 patients, with an average of 10-12 patient visits per day.
- Utilize a dedicated care team, including a medical assistant, scheduler, and referral specialist, to deliver coordinated care and support to patients.
- Collaborate with clinical leadership to implement best practices for patient safety, satisfaction, and overall health improvement.
- Perform office procedures such as suturing, joint injections, skin biopsies, and basic wound care, as appropriate to your training and scope of practice.
- Participate in a rotating call schedule, determined by the number of physicians in the market.
- Engage in activities and programming that promote wellness, such as exercise and art classes, to enhance patients’ quality of life.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM) (required).
- Current, unrestricted, and active license to practice medicine in Arizona.
- Current DEA registration.
- Proficiency in core office procedures, including suturing, joint injections, skin biopsies, and basic wound care.
- Fellowship training in Geriatrics is a plus, but not required.
