Primary Care Physician (MD/DO) – Summary
As a Primary Care Physician (MD/DO), you will provide outpatient primary care to a panel of senior patients, many with complex chronic conditions such as heart disease, diabetes, and hypertension. You will work in a collaborative, multidisciplinary environment, supported by a dedicated care team that includes a medical assistant, scheduler, and referral specialist. Our care model emphasizes continuity, preventive health, and value-based outcomes, ensuring patients receive attentive and coordinated care.
Duties & Responsibilities
- Deliver comprehensive, patient-centered primary care in an outpatient setting to senior patients with multiple chronic conditions.
- Manage a panel of up to 650 patients, averaging 10-12 patients per day for optimal care and relationship building.
- Collaborate with a personal care team, including medical assistants, schedulers, and referral specialists, as well as leadership support.
- Participate in a value-based care model, focusing on patient outcomes, satisfaction, and reducing hospitalizations.
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care as needed.
- Adhere to standard work hours, Monday through Friday, 8:00 a.m. to 5:00 p.m., with a call schedule determined by the provider team size.
- Contribute to the achievement of patient satisfaction goals, HEDIS compliance, and company values.
- Document all patient encounters and care plans in the electronic health record system.
- Stay current with best practices in primary care and participate in ongoing professional development.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree required.
- Board certified or board eligible in Family Medicine (FM) or Internal Medicine (IM).
- Current, unrestricted, and active medical license to practice medicine.
- Current DEA registration required.
- Experience with office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care preferred.
- Fellowship training in Geriatrics is a plus but not required.
- Proficient computer skills and experience with electronic health records.
