Our partnership track typically allows eligible providers to become partners within 12-18 months, with the majority achieving this milestone after just one year. Primary Care Physician (MD/DO) – Summary- Deliver comprehensive primary care services to a senior patient population in an outpatient, value-based care environment.
- Full-time position with a focus on quality, continuity, and preventive health.
- Serve as a clinical leader, collaborating with a multidisciplinary team.
- Manage a small patient panel (up to 450 patients) to ensure personalized, high-quality care.
- See an average of 18 patients per day, allowing for longer, more meaningful visits.
- Utilize dedicated support staff, including a medical assistant, scheduler, and referral specialist.
- Participate in value-based care initiatives, focusing on improved outcomes for seniors with multiple chronic conditions.
- Coordinate care with in-house consulting specialists and holistic health service providers.
- Document patient encounters, develop care plans, and manage follow-ups efficiently with administrative time built into your schedule.
- Support patient access through clinic services such as complimentary transportation.
