Primary Care Physician (MD/DO) - Summary
- Full-time, outpatient-based position focused on providing primary care to seniors

Duties & Responsibilities
- Deliver high-quality, value-based primary care to a panel of approximately 450 patients, primarily seniors with multiple chronic conditions
- Manage an average of 18 patients per day
- Collaborate with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist
- Conduct comprehensive assessments, develop personalized care plans, and coordinate care with in-house consulting specialists (e.g., Cardiology, Podiatry)
- Participate in holistic health services and value-based initiatives such as acupuncture, Tai Chi, yoga, and nutrition counseling
- Utilize administrative time built into your schedule for documentation, care coordination, and ongoing professional development
- Maintain clear communication with patients, families, and interdisciplinary team members
- Engage in continuous improvement activities to enhance patient outcomes and clinic performance
- Uphold the mission to honor seniors by providing compassionate, comprehensive care

Qualifications & Requirements
- MD or DO degree from an accredited institution
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in IM, FM, or Geriatric Medicine
- Commitment to value-based care models and senior patient populations
- Strong communication, organizational, and collaborative skills
- Eligibility to practice medicine in the United States
