Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led, mission-driven primary care organization in Cleveland, OH. As a full-time team member, you will play a pivotal role in delivering high-quality, value-based outpatient care to a senior population, helping to transform the healthcare experience for some of the community’s most vulnerable individuals. This position offers an environment where providers can focus on patient care, supported by a collaborative care team and robust administrative resources.
Responsibilities
- Provide comprehensive primary care services to a panel of senior patients with complex health needs, primarily managing chronic diseases in an outpatient setting.
- See an average of 18 patients per day, ensuring each patient receives personalized attention and evidence-based care.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to optimize patient outcomes and streamline workflows.
- Engage in value-based care initiatives, focusing on improving health outcomes and patient satisfaction.
- Participate in regular team meetings and contribute to the practice’s continuous improvement efforts.
- Utilize in-house consulting specialists (such as cardiology and podiatry) and holistic health services as part of a comprehensive approach to patient wellness.
- Maintain thorough and timely documentation in compliance with regulatory and organizational standards.
- Allocate scheduled administrative time to manage patient records, care coordination, and communication with multidisciplinary team members.
- Champion the organization’s mission to honor seniors with affordable, VIP-level care that enhances their quality of life.
Qualifications
- Medical degree (MD or DO) and successful completion of an accredited residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to value-based, patient-centered care, ideally with experience in managing complex, chronic conditions in a senior population.
- Excellent communication and interpersonal skills, with the ability to work effectively in a collaborative, multidisciplinary environment.
Employment Type and Schedule
- Full-time position based in Cleveland, OH.
- Monday through Friday clinic schedule, with hours from 7:30 a.m. to 5:00 p.m. (last patient scheduled at 4:15 p.m.).
- No night or weekend shifts required, supporting a healthy work-life balance.
- Outpatient-only practice; inpatient responsibilities are not required.
