Primary Care Physician (MD/DO) - Summary
- Provide comprehensive outpatient care for adult and senior patients with multiple chronic diseases.
- Work within a collaborative, multidisciplinary care team dedicated to improving health outcomes for underserved populations.
- Leverage a small patient panel for individualized attention and continuity of care.

Duties & Responsibilities
- Diagnose and treat acute and chronic medical conditions in an outpatient clinic setting.
- Manage a panel of approximately 450 patients, scheduling up to 18 patients per day to ensure personalized care.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to optimize patient outcomes.
- Utilize value-based care approaches to improve patient health and satisfaction.
- Participate in team meetings, quality improvement initiatives, and ongoing professional development.
- Coordinate with in-house consulting specialists such as cardiology and podiatry as needed.
- Provide administrative input and manage electronic health records during scheduled administrative time.
- Engage patients in holistic health services, including acupuncture, Tai Chi, yoga, and nutrition programs available on-site.
- Ensure appropriate referrals, follow-ups, and comprehensive care planning for complex cases.
Salary & Benefits
- Competitive base salary starting at $250,000, with additional performance-based incentive bonuses.
- Potential for partnership with no buy-in, typically achieved within 12-18 months; 65% of physicians attain partner status after one year.
- Signing bonus up to $50,000.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Demonstrated commitment to compassionate, patient-centered care for seniors and adults with complex chronic conditions.
- Strong communication, organizational, and teamwork skills.
- Proficiency in electronic health records and value-based care principles is preferred.

- Clinic hours are Monday through Friday, 7:30 AM to 5:00 PM, with no required nights or weekend shifts.
