Primary Care Physician (MD/DO) – Summary
- Provide high-quality, value-based primary care to a panel of senior patients in an outpatient clinic setting
- Work collaboratively with a dedicated care team, including support staff and in-house specialists
- Participate in a mission-driven organization focused on transforming healthcare for seniors
Duties & Responsibilities
- Deliver comprehensive primary care services to a maximum panel of 450 patients, focusing on seniors with multiple chronic conditions
- Conduct patient consultations, preventive health assessments, diagnosis, and management of chronic illnesses
- Coordinate care with in-house specialists, including cardiology and podiatry, as well as holistic health providers
- Utilize value-based care principles to optimize patient outcomes and healthcare efficiency
- Maintain accurate medical records and participate in quality improvement initiatives
- Leverage a dedicated team (medical assistant, scheduler, referral specialist) to ensure seamless patient care
- Attend to administrative duties during scheduled time to support work-life balance
- Participate in regular interdisciplinary meetings and contribute to team-based care planning
Salary & Benefits
- Competitive base salary starting at $250,000, with incentive bonuses and a signing bonus opportunity
- Potential for partnership, typically achievable within 12-18 months with no buy-in required

Qualifications & Requirements
- Medical degree (MD or DO) with completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certification or Board Eligibility in FM, IM, or Geriatric Medicine
- Dedication to value-based care and improving health outcomes for senior patients
- Ability to manage a moderate patient panel and work collaboratively in a team setting
