Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) for a full-time, outpatient role in Louisville, KY. Key Responsibilities
- Provide comprehensive primary care services to a panel of up to 450 patients, primarily seniors managing multiple chronic conditions.
- Deliver patient-centered, value-based care in a 100% outpatient setting.
- Conduct thorough patient assessments, develop care plans, and manage ongoing treatment for chronic diseases.
- Collaborate with dedicated care support teams, including a medical assistant, scheduler, and referral specialist, to ensure optimal patient experiences and outcomes.
- Utilize in-house consulting specialists in cardiology, podiatry, and other disciplines to coordinate holistic patient care.
- Leverage holistic health services such as acupuncture, Tai Chi, yoga, and nutrition support to enhance patient wellness.
- Maintain accurate and timely documentation of all patient interactions within the electronic health record (EHR) system.
- Participate in quality improvement initiatives and value-based care programs aimed at improving health outcomes for the senior population.
- Adhere to a Monday through Friday working schedule, with clinic hours from 7:30 a.m. to 5:00 p.m., and no night or weekend shifts, supporting work/life balance.
- Ensure administrative time is integrated into the schedule for effective patient management and coordination.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to high-quality, patient-centered care.
- Strong interpersonal and communication skills suitable for working with a diverse senior patient population.
- Experience or interest in value-based care models is highly desirable.
