Primary Care Physician (MD/DO) - Summary
- Physician-led, mission-driven primary care organization

Duties & Responsibilities
- Provide comprehensive primary care to seniors, managing 3-5 chronic diseases on average per patient
- Conduct thorough assessments, develop and implement individualized care plans
- See approximately 18 patients per day in a 100% outpatient setting
- Work closely with dedicated support staff, including a medical assistant, scheduler, and referral specialist
- Collaborate with in-house consulting specialists in cardiology, podiatry, and other relevant disciplines
- Incorporate holistic health approaches such as nutrition, yoga, and Tai Chi into patient care as appropriate
- Engage in value-based care practices to improve patient outcomes
- Maintain accurate, timely documentation and participate in quality improvement initiatives
- Participate in administrative and care coordination activities as included in scheduled work hours
- Contribute to a positive clinical culture and uphold the organization’s mission and values
Salary & Benefits
- Competitive base salary of $250,000+ with additional incentive bonuses
- Signing bonus available (up to $50,000)

- Continuing medical education support ($3,500 + 1 week CME)

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine
- Valid and unrestricted medical license
