Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to join our mission-driven team in Louisville, KY on a full-time basis. This position offers an opportunity for experienced clinicians to make a significant impact on the lives of patients while working within a supportive and collaborative care model.
Key Responsibilities
- Provide high-quality primary care services to a panel of senior patients, focusing on managing and treating multiple chronic diseases within an outpatient setting.
- Maintain a small patient panel with a maximum of 450 patients, ensuring personalized attention and continuity of care.
- Conduct an average of 18 patient visits per day, with administrative time included in your daily schedule to support work/life balance.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to deliver coordinated patient care.
- Utilize a value-based care approach to improve health outcomes and patient satisfaction.
- Participate in partnership opportunities, with many physicians achieving partner status within 12-18 months based on performance and contribution.
- Interact with in-house consulting specialists in fields such as cardiology and podiatry to provide comprehensive care to patients.
- Work closely with other healthcare professionals to deliver holistic health services, including acupuncture, tai chi, yoga, and nutrition programs.
- Ensure compliance with all clinical policies and regulatory standards.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to providing high-quality, compassionate care to a senior population.
- Strong communication and collaboration skills, with the ability to work effectively within an interdisciplinary team.
- Dedication to practicing in a value-based care model and embracing the organization’s mission and values.
Employment Type and Work Schedule
- Full-time position based in Louisville, KY.
- Monday through Friday work schedule, with clinic hours from 7:30 a.m. to 5:00 p.m.; last patient scheduled at 4:15 p.m.
- No nights or weekends required.
- 100% outpatient care environment.
