Primary Care Physician (MD/DO) - Summary- Full-time, outpatient-only primary care position in Louisville, KY
- Focus on providing comprehensive care to a senior patient population
- Work with a supportive, mission-driven team dedicated to value-based healthcare
- Manage a panel of up to 450 patients, primarily seniors with multiple chronic conditions
- Conduct patient visits (average of 18 per day) focusing on preventive care and chronic disease management
- Collaborate with a dedicated support team, including a medical assistant, scheduler, and referral specialist
- Utilize in-house consulting specialists (e.g., Cardiology, Podiatry) for coordinated care
- Engage patients in holistic health services such as acupuncture, Tai Chi, yoga, and nutrition counseling
- Participate in value-based care initiatives aimed at improving outcomes and reducing healthcare disparities
- Maintain thorough documentation and contribute to quality improvement initiatives
- Enjoy a Monday-Friday schedule with clinic hours from 7:30 a.m. to 5:00 p.m., and no nights or weekends
- Competitive base salary with additional incentive bonus opportunities
- Signing bonus up to $50,000

- MD or DO degree from an accredited medical school
- Residency training in Internal Medicine (IM) or Family Medicine (FM) required
- Board certified or board eligible in FM, IM, or Geriatric Medicine
