Primary Care Physician (MD/DO) – Summary
As a Primary Care Physician (MD/DO), you will lead the delivery of value-based, outpatient medical care to a senior patient population, many of whom are managing multiple chronic conditions. You will work closely with a dedicated care team to coordinate and optimize patient outcomes, ensuring each individual receives personalized, comprehensive attention. This position offers the advantage of a regular weekday schedule and a focus on preventive, holistic care.
Duties & Responsibilities
- Provide comprehensive primary care for a panel of up to 450 patients, focusing on seniors with complex health needs.
- Conduct thorough patient evaluations, preventive care, and chronic disease management.
- Collaborate with a multidisciplinary care team, including Medical Assistants, Schedulers, and Referral Specialists, to ensure coordinated care delivery.
- Utilize value-based care principles to improve patient outcomes and quality of life.
- Maintain accurate, up-to-date medical records and documentation.
- Participate in regular administrative time allocated within your schedule for charting and care coordination.
- Refer patients as needed to in-house consulting specialists in areas such as Cardiology and Podiatry.
- Contribute to the ongoing improvement of clinical programs and processes.
- Promote holistic health services, including nutrition, acupuncture, Tai Chi, and yoga as part of overall patient wellness.
Salary & Benefits
- Competitive base salary of $250,000 or more, with additional incentive bonuses available.
- Potential for a significant signing bonus.
- Partnership opportunity typically available within 12-18 months, with no buy-in required.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatrics.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to value-based, patient-centered care for seniors with chronic conditions.
- Ability to collaborate effectively within a multidisciplinary team.
- Spanish language skills preferred but not required.
