Primary Care Physician (MD/DO) - Summary

Duties & Responsibilities
- Provide comprehensive primary care to a small patient panel (maximum 450 patients), focusing on seniors with multiple chronic conditions
- Conduct patient visits, managing approximately 18 patients per day in an outpatient clinic setting
- Develop and implement individualized care plans that address both acute and chronic issues
- Collaborate closely with in-house consulting specialists, including cardiology and podiatry, and a dedicated care team (MA, scheduler, referral specialist)
- Utilize value-based care principles to improve health outcomes and quality of life for your patients
- Participate in holistic health services as appropriate, such as acupuncture, Tai Chi, yoga, and nutrition counseling
- Maintain thorough, accurate documentation and leverage allotted administrative time during scheduled hours
- Engage in ongoing professional development and contribute to the organization’s mission to honor and serve seniors
- Ensure compliance with all clinical, legal, and ethical standards of practice
Salary & Benefits
- Competitive base salary ($250,000+), with performance-based incentive bonuses
- Signing bonus up to $50,000
- Partnership opportunities typically available within 12-18 months, with no buy-in required

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree
- Residency training in Internal Medicine (IM) or Family Medicine (FM) required
- Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Spanish language skills preferred, but not required
- Commitment to value-based, patient-centered care and serving a senior population with complex healthcare needs
