Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) for a full-time opportunity in Philadelphia, PA. As part of a physician-led, mission-driven organization, you will help transform healthcare for seniors, delivering affordable and high-quality care through a value-based approach. Responsibilities
- Provide comprehensive primary care to a panel of up to 450 patients, focusing on seniors with multiple chronic conditions.
- Manage patient care in an outpatient setting, ensuring continuity and quality of care through evidence-based practices.
- Conduct thorough medical assessments, develop individualized treatment plans, and monitor patient outcomes.
- Collaborate with a dedicated clinical support team, including a Medical Assistant, Scheduler, and Referral Specialist, to optimize patient care and workflow efficiency.
- Utilize value-based care principles to achieve better health outcomes for patients.
- Maintain a manageable patient load of approximately 18 patients per day to prioritize quality over volume.
- Participate in administrative activities and maintain accurate, timely medical records.
- Engage with consulting specialists available onsite, such as Cardiology and Podiatry, to support patient needs.
- Offer guidance on holistic health services, including nutrition, acupuncture, tai chi, and yoga, as available within the clinic.
- No nights or weekends required; maintain a Monday through Friday schedule with clinic hours from 7:30am to 5:00pm.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Passion for providing care to the senior population, particularly those with multiple chronic illnesses.
- Strong interpersonal, communication, and organizational skills.
- Spanish language proficiency is preferred but not required.
