Primary Care Physician (MD/DO) - Summary
- Full-time, outpatient-based primary care role
- Serve a senior patient population with complex health needs

Duties & Responsibilities
- Provide primary medical care to a panel of up to 450 senior patients, averaging 18 patients per day
- Develop individualized treatment plans addressing multi-chronic conditions
- Utilize a value-based care model for improved patient outcomes
- Collaborate with in-house consulting specialists (e.g., cardiology, podiatry) and holistic health services including acupuncture, Tai Chi, yoga, and nutrition counseling
- Work closely with a dedicated care team, including a medical assistant, scheduler, and referral specialist
- Maintain thorough documentation and participate in quality assurance initiatives
- Participate in administrative time built into your weekly schedule
- Provide care exclusively during clinic hours (Monday – Friday, 7:30 am – 5:00 pm; last patient at 4:15 pm)
- Participate in organizational efforts to enhance patient access, engagement, and satisfaction

Qualifications & Requirements
- MD or DO degree with completion of residency in Internal Medicine, Family Medicine, or Geriatric Medicine
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine
- Commitment to value-based, patient-centered care
- Excellent communication, organizational, and teamwork skills
- Spanish language proficiency preferred
- Eligibility to work in the United States and hold a valid, unrestricted medical license
