Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated Primary Care Physician (MD/DO) to join our physician-led, mission-driven healthcare organization in Philadelphia, PA. As a Primary Care Physician, you will play a pivotal role in delivering high-quality, value-based outpatient care to a senior patient population, many of whom live with multiple chronic conditions. This full-time position offers a structured Monday through Friday schedule, a supportive multidisciplinary team, and the chance to make a meaningful impact in the community.
Responsibilities
- Deliver comprehensive outpatient primary care services to a small, manageable patient panel, focusing on seniors managing three to five chronic diseases.
- Conduct patient consultations, examinations, and follow-ups, ensuring continuity of care and adherence to value-based care principles.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to optimize patient outcomes and administrative efficiency.
- Utilize in-house consulting specialists such as cardiology and podiatry to enhance patient care as needed.
- Incorporate holistic health services including acupuncture, Tai Chi, yoga, and nutrition education into patient care plans when appropriate.
- Participate in administrative time built into the work schedule, allowing for effective care coordination and professional development.
- Maintain accurate and timely patient records in compliance with all regulatory standards and best practices.
- Advocate for patients by facilitating access to supportive services, including complimentary door-to-door transportation for those in need.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Successful completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to value-based care and improving the quality of life for senior patients.
- Ability to communicate effectively and compassionately with a diverse patient population.
- Spanish language proficiency is preferred, but not required.
