As a Primary Care Physician (MD/DO), you will provide comprehensive outpatient care for a small patient panel of seniors, many managing multiple chronic conditions. Primary Care Physician (MD/DO) – Summary
- Full-time outpatient position providing primary care to a senior patient population
- Focus on value-based care and long-term health outcomes

Duties & Responsibilities
- Deliver comprehensive primary care to a panel of up to 450 patients, primarily seniors with complex chronic conditions
- Manage an average of 18 patients per day in a 100% outpatient setting
- Conduct thorough assessments, develop individualized care plans, and monitor ongoing treatment
- Collaborate with in-house consulting specialists (e.g., cardiology, podiatry) and holistic health services (acupuncture, Tai Chi, yoga, nutrition)
- Lead a dedicated care team including a medical assistant, scheduler, and referral specialist
- Participate in quality initiatives and engage in value-based care practices
- Utilize administrative time built into your schedule for documentation and care coordination
- Maintain compliance with all clinical policies and regulatory standards
- Uphold the organization’s mission to provide VIP-level care for underserved senior populations
- No night or weekend shifts required; enjoy work-life balance
- Foster strong, ongoing relationships with patients, their families, and the care team

Qualifications & Requirements
- Medical degree (MD or DO) with completed residency training in Family Medicine, Internal Medicine, or Geriatric Medicine
- Board certified or board eligible in FM, IM, or Geriatric Medicine
- Commitment to value-based care and population health management
- Spanish language proficiency preferred, but not required
- Strong interpersonal, organizational, and communication skills
- Eligible for medical licensure in Pennsylvania
