Primary Care Physician (MD, DO) Job Overview
We are seeking a dedicated and mission-driven Primary Care Physician (MD, DO) to join our physician-led organization in Atlanta, GA. This is a full-time, outpatient position focused on providing comprehensive, patient-centered care in a collaborative clinical environment. Key Responsibilities
- Deliver high-quality primary care to a panel of up to 450 patients, primarily seniors managing multiple chronic conditions.
- Conduct patient visits, averaging 18 patients per day, ensuring thorough evaluation and management of health concerns.
- Work collaboratively with a dedicated support team, including a medical assistant, scheduler, and referral specialist, to optimize patient outcomes.
- Participate in a value-based care model, focusing on preventive care, chronic disease management, and holistic wellness.
- Coordinate with in-house consulting specialists in cardiology, podiatry, and other fields to provide comprehensive care solutions.
- Incorporate holistic health services such as acupuncture, tai chi, yoga, and nutrition counseling into patient care plans as appropriate.
- Utilize administrative time built into the schedule to complete documentation and coordinate care.
- Maintain a Monday through Friday work schedule with clinic hours from 7:30 AM to 5:00 PM, ensuring work/life balance with no nights or weekends required.
- Leverage door-to-door transportation services provided to patients who require assistance accessing care.
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine.
- Demonstrated commitment to patient-centered, value-based care and continuous quality improvement.
- Strong interpersonal and communication skills, with the ability to work collaboratively as part of a multidisciplinary care team.
