As a Primary Care Physician (MD/DO), you will collaborate with a multidisciplinary team to manage a small patient panel and deliver comprehensive, patient-centered care. Primary Care Physician (MD/DO) - Summary
- Full-time opportunity to provide comprehensive outpatient primary care services.

Duties & Responsibilities
- Conduct thorough patient assessments, develop treatment plans, and provide ongoing management for a panel of up to 450 patients.
- See an average of 18 patients per day in a 100% outpatient setting, ensuring high-quality care and continuity.
- Collaborate with dedicated care teams, including your own medical assistant, scheduler, and referral specialist, to optimize patient outcomes and streamline workflow.
- Utilize value-based care practices and evidence-based guidelines to address the unique needs of a senior population with 3-5 chronic conditions.
- Maintain a regular Monday through Friday schedule (7:30am-5:00pm, last patient scheduled at 4:15pm) with no nights or weekends required.
- Participate in regular administrative time built into your schedule to ensure work-life balance and effective patient management.
- Refer and coordinate in-house specialty consults with cardiology, podiatry, and other specialists as needed.
- Support holistic health initiatives, including access to acupuncture, Tai Chi, yoga, and nutrition services for patients.
- Work collaboratively to achieve organizational goals and participate in quality improvement initiatives.

Qualifications & Requirements
- Medical degree (MD or DO) with completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board certification or eligibility in FM, IM, or Geriatric Medicine.
- Commitment to providing compassionate, patient-centered care to seniors and individuals managing chronic conditions.
- Experience working in outpatient, value-based care settings is a plus.
- Excellent communication, teamwork, and organizational skills.
