Primary Care Physician (MD/DO) - Summary
- Full-time outpatient primary care role serving a senior patient population
- Physician-led, mission-driven healthcare organization focused on value-based care
- Small patient panels for personalized and comprehensive care (up to 450 patients)
- Collaborate with a dedicated support team, including a medical assistant, scheduler, and referral specialist
- Access to in-house specialists and holistic health services
Duties & Responsibilities
- Provide outpatient primary care to seniors, managing 3-5 chronic diseases per patient
- See an average of 18 patients per day in a supportive clinical environment
- Work Monday through Friday, with clinic hours from 7:30 AM to 5:00 PM
- Last patient scheduled at 4:15 PM; no nights or weekends required
- Utilize administrative time built into your schedule for charting and care coordination
- Engage in value-based care initiatives to improve patient outcomes
- Coordinate care with in-house consulting specialists (e.g., Cardiology, Podiatry)
- Participate in holistic health offerings including acupuncture, Tai Chi, yoga, and nutrition counseling
- Work collaboratively with your dedicated support team to ensure seamless patient care
- Participate in partnership track opportunities with no buy-in required, typically achieved within 12-18 months

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Completion of residency in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in IM, FM, or Geriatric Medicine
- Commitment to providing high-quality, value-based care to seniors
- Strong communication, organizational, and teamwork skills
- Ability to work collaboratively within a multidisciplinary team
