Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join a mission-driven primary care organization in Richmond, VA. Key Responsibilities
- Provide comprehensive outpatient primary care services to a dedicated panel of senior patients, typically managing 18 patients per day.
- Deliver value-based care by focusing on prevention, chronic disease management, and coordination of care for patients, many of whom live with multiple chronic conditions.
- Collaborate closely with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist, to ensure seamless patient experiences and optimal health outcomes.
- Work Monday through Friday, with clinic hours from 7:30 a.m. to 5:00 p.m., and no night or weekend shifts required, supporting a healthy work-life balance.
- Engage in administrative activities within scheduled hours, ensuring all documentation and care coordination tasks are effectively managed.
- Participate in interdisciplinary collaboration with in-house consulting specialists in cardiology, podiatry, and additional specialties as needed for comprehensive patient care.
- Support holistic health initiatives, including access to acupuncture, Tai Chi, yoga, and nutrition services offered within the clinic.
- Utilize door-to-door patient transportation services, enhancing access to care for those with mobility challenges in Richmond, VA.
Qualifications
- Medical degree (MD/DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM) or Family Medicine (FM); Geriatric Medicine training is also acceptable.
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to practicing evidence-based, value-driven care for senior populations.
- Excellent communication, teamwork, and organizational skills.
