Primary Care Physician (MD/DO) - Summary- Full-time position focused on outpatient primary care for seniors
- Emphasis on value-based care and patient-centered wellness
- Supportive, physician-led clinical environment
- Opportunities for partnership within 12-18 months (no buy-in required)
- Manage a small patient panel (up to 450 patients), ensuring individualized care
- Conduct comprehensive primary care visits for approximately 18 patients per day
- Provide care to seniors with 3-5 chronic diseases, focusing on prevention and wellness
- Collaborate with a dedicated care team (MA, scheduler, referral specialist)
- Utilize in-house consulting specialists (cardiology, podiatry, and more) for coordinated care
- Integrate holistic health services, including acupuncture, Tai Chi, yoga, and nutrition
- Participate in value-based care initiatives to improve patient outcomes
- Maintain administrative responsibilities during scheduled clinic time
- Work Monday through Friday; no nights or weekends required
- Highly competitive base salary with additional incentive bonuses
- Annual partnership track after 12-18 months (65% achieve partnership after one year)
