Primary Care Physician (MD/DO) OverviewWe are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led, mission-driven healthcare organization in Orlando, FL. As a full-time provider, you will play an essential role in delivering high-quality, value-based primary care to a senior patient population. About the RoleThe Primary Care Physician (MD/DO) will focus on providing outpatient care to a panel of senior patients, many of whom have multiple chronic conditions. This role is designed to foster strong patient-provider relationships through smaller patient panels, allowing for personalized care. You will be supported by a dedicated care team that includes a medical assistant, scheduler, and referral specialist to ensure you can focus on clinical care. The work schedule is Monday through Friday, with no nights or weekends required, and administrative time is included during your workday. As a valued member of our team, you will have opportunities for professional growth, including a partnership track for eligible physicians.
Key Responsibilities- Provide comprehensive primary care to seniors in an outpatient setting, focusing on management of chronic diseases and preventive health.
- Manage a maximum patient panel of 450, typically seeing approximately 18 patients per day.
- Collaborate with in-house specialists, such as cardiology and podiatry, and leverage holistic health services including acupuncture, Tai Chi, yoga, and nutrition support.
- Utilize value-based care models to achieve optimal health outcomes for patients.
- Work as part of a multidisciplinary team, supported by a medical assistant, scheduler, and referral specialist.
- Engage patients in their health care by providing education and encouraging adherence to treatment plans.
- Ensure accurate and timely documentation of patient care in the electronic medical record system.
- Participate in regular team meetings and contribute to quality improvement initiatives.
- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Current and unrestricted medical license to practice as a Physician in the state of Florida, or eligibility to obtain one.
