As a Primary Care Physician (MD/DO), you will collaborate with a multidisciplinary team to provide comprehensive outpatient care for seniors, focusing on patients with multiple chronic conditions. You will benefit from a highly supportive administrative structure, including your own dedicated support staff, to streamline patient care and maximize your impact. Our clinics are equipped with in-house consulting specialists in disciplines such as cardiology and podiatry, and offer holistic health services including acupuncture, tai chi, yoga, and nutrition counseling. Patients benefit from convenient door-to-door transportation, helping to remove barriers to care and improve health outcomes. You will see an average of 18 patients per day, allowing ample time for meaningful interactions and comprehensive care planning. Administrative time is integrated into your schedule, and there are no night or weekend shifts required.

Duties & Responsibilities
- Provide comprehensive primary care to a panel of up to 450 patients, focusing on seniors with 3-5 chronic diseases
- Conduct an average of 18 patient visits per day, ensuring high-quality, individualized care
- Collaborate with a dedicated care team including medical assistants, schedulers, and referral specialists
- Coordinate with in-house consulting specialists (e.g., cardiology, podiatry) for complex cases
- Engage in holistic care practices by integrating wellness services such as acupuncture, yoga, and nutrition counseling into treatment plans
- Participate in value-based care initiatives to improve patient outcomes and organizational performance
- Maintain thorough documentation and utilize administrative time built into the schedule
- Operate within regular clinic hours, Monday through Friday, with no nights or weekends

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Residency training in Internal Medicine (IM) or Family Medicine (FM)
- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
