Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) for a full-time opportunity in Jacksonville, FL. As part of a physician-led, mission-driven primary care organization, you will play a key role in transforming healthcare for seniors and providing value-based, patient-centered care in a supportive environment. Responsibilities
- Deliver comprehensive primary care in an outpatient setting, focusing on the senior population with multiple chronic conditions.
- Manage a small patient panel of up to 450 patients, allowing more personalized and attentive care.
- Provide care to an average of 18 patients per day, ensuring quality interactions and follow-up.
- Collaborate closely with a dedicated support team, including a medical assistant, scheduler, and referral specialist, to optimize patient experience and care coordination.
- Participate in value-based care initiatives, contributing to improved health outcomes and patient satisfaction.
- Utilize scheduled administrative time to manage patient records, referrals, and care plans efficiently.
- Refer patients to in-house consulting specialists as needed, including cardiology and podiatry.
- Engage in holistic health services offered by the clinic, such as acupuncture, Tai Chi, yoga, and nutrition counseling.
- Adhere to clinic hours of Monday through Friday, 7:30 AM to 5:00 PM, with the last patient scheduled at 4:15 PM.
- Maintain work-life balance with no required nights or weekends and a focus on outpatient care.
Qualifications
- MD or DO degree with completion of residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Passion for serving senior populations, particularly those with complex health needs.
- Strong commitment to value-based, patient-centered care and continuous quality improvement.
- Excellent communication, teamwork, and organizational skills.
