Primary Care Physician (MD/DO) - Summary- Full-time, outpatient role focused on senior care

- Provide comprehensive primary care for a panel of up to 450 patients, primarily seniors with multiple chronic conditions
- Conduct patient visits (average 18 per day) in a 100% outpatient setting
- Collaborate with a dedicated care team including medical assistants, schedulers, and referral specialists
- Participate in value-based care initiatives to achieve optimal patient outcomes
- Utilize administrative time included in your daily schedule for patient care planning and documentation
- Coordinate with in-house consulting specialists and holistic health services as needed
