Primary Care Physician (MD/DO) Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led, mission-driven primary care organization. This is a full-time position located in Newport News, VA, focused on providing affordable, high-quality healthcare to seniors and individuals managing multiple chronic conditions. As a Primary Care Physician on our team, you will play a key role in transforming the care experience for our patients and helping fulfill our mission to deliver better health outcomes for our community.
Key Responsibilities
- Deliver comprehensive outpatient primary care to a dedicated patient panel, with a focus on seniors and patients with multiple chronic diseases.
- Maintain a manageable patient panel size (up to 450 patients) to ensure high-quality care and personalized attention.
- Provide care in a value-based model, emphasizing prevention, chronic disease management, and coordination of care with an interdisciplinary team.
- Manage an average of 18 patients per day, supported by a dedicated care team including a Medical Assistant, Scheduler, and Referral Specialist.
- Participate in a Monday through Friday work schedule with clinic hours from 7:30am to 5:00pm; the last patient is scheduled at 4:15pm.
- Utilize administrative time built into your schedule to complete charting, care coordination, and follow-up activities.
- Collaborate with in-house consulting specialists in areas such as Cardiology and Podiatry, and integrate holistic health services including Acupuncture, Tai Chi, Yoga, and Nutrition into patient care plans.
- Work in an environment that values work-life balance, with no nights or weekend shifts required.
- Support transportation needs of patients by coordinating with the clinic's complimentary door-to-door service.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Board Certified or Board Eligible in Family Medicine (FM), Internal Medicine (IM), or Geriatric Medicine.
- Eligibility for medical licensure in the state of Virginia.
- Open to candidates requiring visa sponsorship.
- Demonstrated commitment to value-based care and improving outcomes for seniors and patients with chronic conditions.
