Primary Care Physician (MD/DO) OverviewWe are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our mission-driven, physician-led organization in Hollywood, FL. Responsibilities- Provide comprehensive primary care services to a panel of up to 450 patients, focusing on seniors with multiple chronic conditions.
- Manage an average of 18 patient visits per day in an outpatient clinic setting.
- Collaborate with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to ensure optimal patient outcomes.
- Utilize value-based care principles to improve patient health and achieve organizational quality goals.
- Participate in administrative activities, with dedicated time included in your schedule for documentation and care coordination.
- Coordinate with in-house consulting specialists, such as cardiologists and podiatrists, to address complex patient needs.
- Promote holistic health services, including acupuncture, Tai Chi, yoga, and nutrition counseling, as part of a comprehensive care approach.
- Maintain a Monday through Friday work schedule, with clinic hours from 7:30 a.m. to 5:00 p.m. and no required nights or weekends.
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree.
- Completion of a residency program in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine.
- Commitment to practicing evidence-based medicine with a focus on geriatric populations and complex care management.
- Strong interpersonal and communication skills to effectively work within a collaborative care team.
This is a full-time position based in Hollywood, FL. The practice is 100% outpatient, offering an excellent work/life balance with no night or weekend responsibilities.
