Primary Care Physician (MD/DO) - Summary- Full-time, outpatient primary care physician role (MD/DO)
- Focus on caring for a panel of up to 450 senior patients
- Based in Gretna, LA, serving a diverse senior community

- Deliver comprehensive primary care to seniors with multiple chronic conditions
- Manage a daily patient load of approximately 18 individuals
- Utilize value-based care models to improve patient outcomes and satisfaction
- Coordinate patient care with a dedicated support team (MA, scheduler, referral specialist)
- Participate in partnership opportunities typically available within 12-18 months
- Engage with in-house consulting specialists and holistic health services
- Ensure administrative duties are completed during allocated schedule time
- Maintain excellent work-life balance (no nights or weekends, regular weekday hours)
