Primary Care Physician (MD/DO) - Summary
- Full-time, outpatient-only role focused on comprehensive primary care for seniors

Duties & Responsibilities
- Provide direct primary care services to a maximum panel of 450 patients, primarily seniors with multiple chronic conditions
- Conduct scheduled office visits, averaging 18 patients per day, Monday through Friday
- Perform comprehensive assessments, develop individualized care plans, and manage ongoing treatment for chronic diseases
- Collaborate closely with a dedicated care team, including a medical assistant, scheduler, and referral specialist
- Utilize value-based care principles to improve health outcomes and patient satisfaction
- Maintain accurate and timely documentation in the electronic health record (EHR)
- Participate in care coordination with in-house consulting specialists (such as cardiology and podiatry) and holistic health services (acupuncture, tai chi, yoga, nutrition)
- Allocate dedicated administrative time within your schedule for non-clinical tasks
- Contribute to a positive, collaborative clinic culture that supports continuous quality improvement
- Work exclusively in an outpatient setting with no night or weekend shifts required

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Board certified or board eligible in Family Medicine, Internal Medicine, or Geriatric Medicine
- Eligible for medical licensure
- Open to candidates requiring visa sponsorship
- Strong commitment to value-based, patient-centered care
- Excellent communication, organizational, and teamwork skills
- Ability to manage a complex patient population with multiple chronic conditions
- Experience with electronic health records (EHR) preferred
