Primary Care Physician (MD/DO) Position Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our mission-driven outpatient clinic in Portsmouth, VA. Responsibilities
- Deliver comprehensive primary care services to a panel of adult and senior patients in an outpatient clinic setting.
- Manage a small patient panel of up to 450 individuals, focusing on quality and continuity of care.
- Conduct thorough patient assessments, develop care plans, and monitor ongoing treatment for seniors, many of whom have multiple chronic diseases.
- Work collaboratively with a dedicated clinical support team, including a medical assistant, scheduler, and referral specialist, to coordinate patient care.
- Participate in value-based care initiatives and employ evidence-based practices to improve patient outcomes.
- Refer patients to in-house consulting specialists when appropriate, including cardiology, podiatry, and other specialties.
- Incorporate holistic health services such as acupuncture, tai chi, yoga, and nutrition counseling into patient care plans as needed.
- Allocate administrative time within your schedule to ensure all documentation and care coordination is completed efficiently.
- Adhere to a Monday through Friday clinic schedule, with hours from 7:30 AM to 5:00 PM, and no night or weekend shifts required.
- Promote patient engagement in their health, emphasizing preventive care and education to support long-term wellness.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to patient-centered care, particularly for senior populations with complex health needs.
- Strong communication skills and the ability to work collaboratively within a multidisciplinary care team.
- Ability to manage a diverse caseload and adapt to the evolving needs of patients.
- Open to candidates who require visa sponsorship.
