As a Primary Care Physician (MD/DO), you will provide comprehensive medical care to a panel of senior patients in an outpatient setting. You will work alongside a dedicated care team, including your own Medical Assistant, Scheduler, and Referral Specialist. Our care model emphasizes value-based care, focusing on preventive medicine and the management of chronic conditions to achieve optimal patient outcomes. Support services such as in-house consulting specialists (Cardiology, Podiatry, and more), holistic health services (Acupuncture, Tai Chi, Yoga, and Nutrition), and door-to-door transportation for patients enhance the quality of care delivered.

Primary Care Physician (MD/DO) - Summary
- Full-time, outpatient role focused on adult and senior patient populations
- Provide primary care services in a supportive, team-based environment

Duties & Responsibilities
- Deliver comprehensive primary care to a panel of up to 450 patients, focusing on seniors with chronic conditions
- Conduct patient visits (approximately 18 patients per day) during clinic hours, Monday through Friday (7:30 a.m. – 5:00 p.m.)
- Participate in value-based care initiatives, emphasizing preventive medicine and chronic disease management
- Collaborate with a dedicated support team, including Medical Assistants, Schedulers, and Referral Specialists
- Engage with in-house consulting specialists and leverage holistic health services for patient benefit
- Utilize administrative time included in your schedule for documentation and care coordination
- Foster strong patient relationships and deliver care with empathy, respect, and professionalism
- Maintain compliance with all relevant clinical protocols and standards
- No night or weekend shifts, ensuring a sustainable work-life balance

Qualifications & Requirements
- Medical Degree (MD or DO) from an accredited institution
- Completion of a Residency Program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Commitment to value-based, patient-centered care
- Strong communication, collaboration, and clinical documentation skills
