Primary Care Physician (MD/DO) Position Overview

Key Responsibilities
- Provide comprehensive, patient-centered primary care services in an outpatient clinic setting focused on senior populations, particularly those with multiple chronic conditions.
- Manage a dedicated patient panel of up to 450 individuals to ensure personalized and continuous care.
- Conduct thorough assessments, diagnosis, and management of acute and chronic conditions in accordance with current clinical guidelines.
- Work closely with a dedicated care team, including a Medical Assistant, Scheduler, and Referral Specialist, to optimize care delivery and patient satisfaction.
- Leverage value-based care models to improve patient outcomes, focusing on preventive measures and chronic disease management.
- Coordinate care with in-house consulting specialists and holistic health service providers, including cardiology, podiatry, acupuncture, tai chi, yoga, and nutrition professionals.
- Ensure all administrative responsibilities are managed efficiently with allocated administrative time built into the schedule.
- Participate actively in partnership opportunities, with a clear path to partnership typically available within 12-18 months.
Qualifications
- Medical degree (MD or DO) from an accredited institution.
- Completion of residency training in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Demonstrated commitment to providing high-quality, compassionate care to diverse senior populations.
- Strong communication, organizational, and teamwork skills.
Employment Details and Benefits
- Full-time, outpatient position with a Monday through Friday schedule (7:30 a.m. – 5:00 p.m.; last patient scheduled at 4:15 p.m.).
- No night or weekend shifts required, supporting a healthy work-life balance.
