The Primary Care Physician (MD/DO) role is supported by a dedicated care team including a medical assistant, scheduler, and referral specialist, allowing you to focus on providing comprehensive, proactive care. Our patient population consists primarily of seniors with multiple chronic health conditions, and our clinic offers in-house consulting specialists and holistic health services to support a well-rounded approach to wellness. We also provide door-to-door transportation for patients to reduce barriers to care and ensure consistent access to services.

Primary Care Physician (MD/DO) - Summary
- Deliver high-quality, value-based primary care for a panel of senior patients managing multiple chronic conditions.

Duties & Responsibilities
- Manage a small patient panel (up to 450 patients), focusing on quality over quantity with approximately 18 patients per day.
- Perform comprehensive health assessments, develop and implement individualized care plans, and coordinate care across specialties as needed.
- Collaborate with an in-house team, including consulting specialists in cardiology, podiatry, and holistic health services such as acupuncture, Tai Chi, yoga, and nutrition.
- Utilize value-based care principles to optimize patient outcomes and satisfaction.
- Participate in regular team meetings and quality improvement initiatives.
- Maintain thorough and accurate electronic health records.
- Engage in ongoing professional development and training to remain current with best practices in internal medicine, family medicine, and geriatrics.
- Adhere to all clinical and regulatory guidelines and standards.

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution.
- Board Certification or Board Eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.

- Experience managing complex patients with multiple chronic diseases preferred.
- Ability to work effectively in a multidisciplinary team environment.
