Primary Care Physician (MD/DO) Position OverviewWe are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our physician-led, mission-driven organization on a full-time basis in Lakeland, FL. Our organization is committed to transforming healthcare delivery for seniors by providing affordable, VIP-level care that results in better health outcomes. As a Primary Care Physician (MD/DO) in Lakeland, FL, you will play a critical role in delivering value-based care and making a meaningful impact in the lives of patients with complex health needs.
Key Responsibilities- Manage a small patient panel of up to 450 seniors, focusing on individuals with 3-5 chronic diseases.
- See an average of 18 patients per day in a 100% outpatient setting.
- Deliver comprehensive primary care using a value-based model to maximize patient health and well-being.
- Coordinate care with a dedicated support team, including a medical assistant, scheduler, and referral specialist.
- Utilize administrative time included in your schedule to ensure thorough patient documentation and follow-ups.
- Collaborate with in-house consulting specialists, such as cardiology and podiatry, to deliver holistic patient care.
- Participate in holistic health offerings including acupuncture, tai chi, yoga, and nutrition services for patients.
- Ensure all care is provided during regular clinic hours: Monday through Friday, 7:30 a.m. to 5:00 p.m. (last patient scheduled at 4:15 p.m.), with no night or weekend shifts required.
- Support the organization's mission to honor seniors and achieve positive health outcomes.
- Medical degree (MD or DO) from an accredited institution.
- Completion of an accredited residency program in Internal Medicine (IM), Family Medicine (FM), or Geriatric Medicine.
- Board Certified or Board Eligible in FM, IM, or Geriatric Medicine.
- Strong commitment to value-based care and improving health outcomes for senior populations.
- Excellent communication and teamwork skills.
