Primary Care Physician (MD/DO) - Summary- Full-time role providing comprehensive primary care to a senior patient population
- Mission-driven, physician-led organization focused on value-based health outcomes
- Supportive work environment with dedicated administrative and clinical resources

- Manage a small patient panel (maximum of 450 patients) to ensure personalized care
- Conduct outpatient visits, averaging 18 patients per day, with administrative time built into your schedule
- Provide care Monday through Friday, 7:30am to 5:00pm, with no nights or weekends required
- Collaborate with a dedicated care team, including your own medical assistant, scheduler, and referral specialist
- Coordinate care for seniors with 3-5 chronic conditions, utilizing a value-based approach
- Leverage in-house consulting specialists, such as cardiology and podiatry, to support comprehensive care
- Participate in holistic health services, including acupuncture, Tai Chi, yoga, and nutrition programs
- Document and update patient records accurately and efficiently
- Engage in regular team meetings and continued education to uphold best clinical practices
- Competitive base salary with bonus potential based on experience and performance

- Medical degree (MD or DO) with completion of residency training in Internal Medicine, Family Medicine, or Geriatric Medicine
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics

- Strong communication, teamwork, and organizational skills
- Dedication to continuous professional development and alignment with value-based care principles
- Experienced or early-career primary care physician passionate about senior care
- Proven ability to manage complex, chronic conditions in an outpatient setting
