Primary Care Physician (MD/DO) Overview
We are seeking a dedicated Primary Care Physician (MD/DO) for a full-time opportunity in Bradenton, FL. Responsibilities
- Deliver comprehensive primary care to a panel of up to 450 senior patients, focusing on those with multiple chronic conditions.
- Conduct patient visits in a 100% outpatient setting, with no night or weekend shifts required.
- See an average of 18 patients per day during clinic hours, which run Monday through Friday from 7:30 a.m. to 5:00 p.m., with the last patient scheduled at 4:15 p.m.
- Work collaboratively with a dedicated care team, including a medical assistant, scheduler, and referral specialist, to ensure optimal patient outcomes.
- Participate in value-based care delivery, focusing on preventive medicine and chronic disease management.
- Utilize designated administrative time within the schedule to complete patient documentation and coordinate care plans.
- Access to in-house consulting specialists in areas such as cardiology and podiatry, as well as holistic health services like acupuncture, tai chi, yoga, and nutrition counseling.
- Contribute to a supportive and collaborative clinic environment dedicated to improving the health and well-being of the local senior community.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) degree from an accredited institution.
- Completed residency training in Internal Medicine (IM) or Family Medicine (FM).
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatric Medicine.
- Commitment to value-based, patient-centered care, and improving health outcomes for seniors.
- Strong interpersonal and communication skills, with the ability to work effectively as part of a multidisciplinary care team.
