Primary Care Physician (MD/DO) - Summary- Provide comprehensive, value-based care to Medicare patient populations
- Serve as part of a multidisciplinary team in a neighborhood primary care center
- Leverage advanced EMR systems and onsite laboratory resources
- Focus on preventive health, wellness, and patient education initiatives
- Promote a culture of quality, safety, and continuous improvement
- Conduct patient examinations, develop care plans, and provide ongoing management for a panel of Medicare patients
- Collaborate with scribes, medical assistants, social workers, and other clinical staff to deliver coordinated care
- Utilize onsite labs and specialty consultation access for comprehensive diagnostics
- Maintain accurate, timely documentation within Canopy and Greenway EMR systems
- Participate in value-based care models, focusing on preventive care and reduction of hospital admissions
- Engage in patient and community wellness activities, including educational events and group sessions
- Ensure compliance with all legal, regulatory, and organizational standards

- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
- Board certification or eligibility in Internal Medicine or Family Medicine
- Active, unrestricted state medical license
- Bilingual (Spanish-speaking) preferred
- No residents; seeking fully licensed physicians
- Fellowship training in Geriatrics preferred but not required
- Experienced in value-based care models
- Patient-focused with strong communication and collaboration skills
- Adaptable, innovative, and committed to continuous learning
- Demonstrates cultural competency and a passion for serving diverse populations
- Fluency in Spanish is a significant asset

- Average patient volume: 13-16 per day
- Appointment length: 20 minutes for regular visits, 40 minutes for new or full diagnostic
- Access to onsite laboratory services and specialty consults
