Primary Care Physician (MD/DO) About the Opportunity
This full-time Primary Care Physician (MD/DO) position in Denver, CO, offers a unique chance to deliver value-based healthcare in an innovative primary care clinic network. As a Primary Care Physician, you will join a dedicated team focused on providing high-quality, equitable healthcare to Medicare patient populations. Our clinics are designed to prioritize patient outcomes and preventive medicine, helping to reduce hospital admissions and enhance the patient experience through a supportive, team-based care model.
Key Responsibilities
- Deliver comprehensive primary care services to a diverse Medicare patient base, focusing on preventive care and chronic disease management.
- Work collaboratively with a multidisciplinary care team, including scribes, medical assistants, and social workers, to ensure coordinated and effective patient care.
- Utilize value-based care principles to drive positive health outcomes and contribute to the reduction of avoidable hospital admissions.
- Manage a patient volume of 13-16 patients per day, with scheduled visits including 20-minute routine appointments and 40-minute diagnostic or new patient consultations.
- Leverage advanced clinic resources, such as onsite laboratory services with a dedicated phlebotomist and access to tele-specialist consultations through RubiconMD.
- Document all patient encounters accurately using electronic medical record (EMR) systems, including Canopy and Greenway.
- Participate in and support community-based patient activities and wellness programs offered through clinic facilities.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or board eligibility in Internal Medicine or Family Medicine.
- Active, non-probationary, and unrestricted state medical license.
- Fellowship training in Geriatrics is preferred but not required.
- Residency training completed; this position is not open to current residents.
Work Hours and Clinic Environment
- Full-time, Monday through Friday, 8:00am – 5:00pm schedule.
- Average of 3-5 providers per clinic, with a mix of Nurse Practitioners (NPs) and Primary Care Physicians (MD/DO).
