Primary Care Physician (MD/DO) – Summary- Full-time opportunity focused on value-based care for Medicare populations
- Collaborative, multidisciplinary team environment
- Emphasis on preventive and patient-centered medicine
- Deliver comprehensive primary care to adult and senior patients within a value-based care framework
- Maintain an average daily patient volume of 13–16, with 20-minute routine visits and 40-minute diagnostic/new patient appointments
- Work collaboratively with care teams, including scribes, medical assistants, and social workers
- Utilize onsite laboratory resources and specialist telehealth consultations (RubiconMD)
- Participate in care coordination and patient outreach initiatives
- Contribute to a positive patient experience, supporting wellness activities and community engagement
- Document patient encounters and care plans using EMR systems (Canopy and Greenway)

- Board certified or board eligible in Internal Medicine or Family Medicine
- Active, non-probationary, unrestricted state medical license
- Fellowship training in Geriatrics preferred but not required
