Primary Care Physician (MD/DO) – Summary
- Deliver outpatient primary care services within a value-based care model
- Work collaboratively with multidisciplinary care teams to optimize patient outcomes
- Focus on preventive medicine, chronic disease management, and reduced hospital admissions
- Utilize electronic medical records (EMR) and onsite diagnostic resources
Duties & Responsibilities
- Conduct comprehensive primary care evaluations for adult and senior Medicare patients
- Manage a daily patient volume of approximately 13-16 patients
- Allocate 20 minutes for standard visits and 40 minutes for full diagnostic or new patient appointments
- Work as part of a care team consisting of a scribe, medical assistant, and social worker
- Coordinate care with onsite laboratory services and utilize specialist access platforms such as RubiconMD
- Engage in community health initiatives, including organizing and supporting wellness activities for patients
- Maintain accurate documentation using Canopy and Greenway EMR systems
- Participate in quality improvement initiatives and value-based care programs

Qualifications & Requirements
- Graduate of an accredited Medical Doctor (MD) or Doctor of Osteopathy (DO) program
- Board certification or board eligibility in Internal Medicine or Family Medicine
- Active, unrestricted State Medical License in good standing
- No current residents accepted for this opportunity
- Fellowship training in Geriatrics preferred, but not required
