Primary Care Physician (MD/DO) Position Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) for a full-time role in Bridgeport, CT. This opportunity is ideal for physicians who are passionate about delivering value-based healthcare to Medicare patient populations. As part of our innovative network of neighborhood primary care centers, you will play a vital role in improving patient outcomes and enhancing the overall patient experience. Our practice model is designed to provide robust support, allowing you to focus on quality care over volume.
Key Responsibilities
- Deliver equitable and effective primary care to Medicare patients within a value-based care framework.
- Work closely within a multidisciplinary care team including scribes, medical assistants, and social workers to coordinate comprehensive care.
- Maintain a manageable daily patient volume of approximately 13-16 patients, ensuring quality of care with 20-minute standard visits and 40-minute diagnostic/new patient visits.
- Utilize on-site lab services and access to RubiconMD for specialist consultations to enhance diagnostic and treatment capabilities.
- Document all patient interactions and care provided using EMR systems including Canopy and Greenway.
- Promote preventive medicine, emphasizing wellness and reducing unnecessary hospital admissions.
- Support community health initiatives by participating in patient activities such as exercise programs, wellness events, and social gatherings in the community room.
- Collaborate with an average of 3-5 providers per clinic, including nurse practitioners and other primary care physicians.
- Contribute to the organization's mission of providing high-value, patient-centered care.
Required Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary state medical license.
- Fellowship training in Geriatrics is preferred but not required.
- Must not currently be in a residency program.
