Primary Care Physician (MD/DO) Position Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our established, innovative neighborhood primary care center in New Haven, CT. This full-time opportunity is an excellent fit for experienced physicians looking to deliver outstanding, equitable, and value-based healthcare to local Medicare patient populations. Our care model focuses on empowering physicians to prioritize patient outcomes and preventive medicine, supported by a robust, multidisciplinary care team. Working as a Primary Care Physician (MD/DO) in New Haven, CT, you will contribute to an environment that values quality care and fosters patient engagement through a collaborative, team-based approach.
Key Responsibilities
- Deliver comprehensive primary care services to a panel of Medicare patients, focusing on prevention, wellness, and chronic disease management.
- Utilize value-based care principles to reduce hospital admissions and improve overall patient health outcomes.
- Work collaboratively with a care team including scribes, medical assistants, and social workers to ensure coordinated, patient-centered care.
- Perform patient visits averaging 13-16 per day, with appointments structured for quality—20 minutes for regular visits and 40 minutes for full diagnostic/new patient assessments.
- Leverage on-site laboratory services and specialist access through platforms like RubiconMD to provide timely diagnostics and consultations.
- Document patient encounters using EMR systems (Canopy and Greenway) and adhere to best practices for electronic health record management.
- Participate in community engagement initiatives, including exercise classes and wellness events, to promote health within the patient population.
- Engage in continuous quality improvement by tracking and achieving quality metrics that contribute to organizational goals and bonus incentive structures.
Qualifications
- Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) from an accredited institution.
- Board Certification or Board Eligibility in Internal Medicine or Family Medicine required.
- Active, unrestricted, and non-probationary medical license in the state of Connecticut.
- Fellowship training in Geriatrics is preferred but not required.
- Ability to work collaboratively in a multidisciplinary team environment.
- Strong commitment to value-based, patient-centered care and continuous improvement.
- Residents are not eligible for this position.
Practice Environment
- Physician Partnership Track available for long-term career growth.
- Support structure including 3-5 providers per clinic, with a mix of Nurse Practitioners and Physicians.
- On-site amenities include laboratory services, patient transportation within 5 miles, and community activity rooms.
