Primary Care Physician (MD/DO) - Summary- Deliver comprehensive, value-based primary care to adult Medicare populations in New Haven, CT
- Work collaboratively within a multidisciplinary care team, including MAs, scribes, and social workers
- Focus on preventive medicine, quality outcomes, and enhanced patient experience
- Utilize advanced technology and on-site laboratory resources
- Provide direct primary care services, including assessments, diagnosis, treatment, and follow-up
- Practice evidence-based medicine in alignment with value-based care principles
- Manage a patient panel with an average daily volume of 13-16 patients
- Allocate appropriate visit times (20 minutes for standard visits, 40 minutes for new or complex cases)
- Collaborate closely with care team members to address medical, behavioral, and social determinants of health
- Engage in preventive health initiatives and patient education
- Utilize EMR systems (Canopy, Greenway) for accurate and timely documentation
- Participate in quality improvement activities and outcomes-based performance measurement
- Support community-based health programs and patient engagement activities
- Contribute to a positive, inclusive, and collaborative clinic culture

- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree
- Board certified or board eligible in Internal Medicine or Family Medicine
- Active, unrestricted State License in good standing
- No current residents
- Fellowship training in Geriatrics preferred but not required
- Demonstrates a passion for delivering high-quality, patient-centered care
- Committed to practicing within a value-based care model
- Collaborative, team-oriented approach to healthcare delivery
- Strong communication and interpersonal skills
- Comfortable utilizing EMR systems and technology in clinical practice
- Interest in community engagement and preventive health initiatives
- Average clinic team includes 3-5 providers (mix of NPs and Primary Care Physicians)
- On-site labs and phlebotomy services enhance workflow and patient convenience
