Primary Care Physician (MD/DO) Job Overview
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative team in Cedar Rapids, IA. This full-time opportunity is ideal for a provider passionate about delivering value-based healthcare to Medicare patient populations within a collaborative and supportive environment. As a Primary Care Physician, you will work alongside a multidisciplinary team with a strong focus on patient outcomes, preventive medicine, and holistic care.
Responsibilities
- Deliver comprehensive primary care services to a diverse panel of Medicare patients, focusing on preventive medicine, chronic disease management, and patient education.
- Collaborate with a dedicated care team that may include scribes, medical assistants, and social workers to ensure seamless, coordinated care for all patients.
- Conduct patient visits averaging 13-16 per day, with appointment times structured for both routine visits and new patient or full diagnostic evaluations.
- Utilize on-site resources such as laboratory services and access to RubiconMD for specialist consultations to enhance patient care.
- Maintain accurate and timely documentation in electronic medical record (EMR) platforms including Canopy and Greenway.
- Support and participate in community health events and patient activities held in the clinic’s community room, such as exercise classes, wellness events, and health education sessions.
- Contribute to the clinic’s value-based care model by focusing on quality metrics, reducing hospital admissions, and enhancing patient satisfaction.
- Engage in continuous professional development and quality improvement initiatives as part of a physician partnership track.
Qualifications
- Graduate of an accredited Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) program.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license required.
- Bilingual proficiency in Spanish is preferred to better serve our patient population.
- Fellowship training in Geriatrics is preferred but not required.
- This position is not open to current residents.
