The Primary Care Physician (MD/DO) will work closely with a robust care team, including scribes, medical assistants, and social workers, to ensure effective and efficient patient management. With access to on-site labs, specialist consultation platforms, and an electronic medical records system, you will have the tools necessary to provide evidence-based preventive and diagnostic care. Primary Care Physician (MD/DO) – Summary
- Provide high-quality, value-based care to Medicare patient populations in a supportive, team-based environment.
- Focus on preventive care, managing chronic conditions, and improving patient outcomes through an innovative care delivery model.
- Work with a collaborative team including scribes, medical assistants, and social workers.
- Utilize advanced EMR systems and on-site laboratory resources for efficient patient management.
Duties & Responsibilities
- Conduct comprehensive medical evaluations, wellness visits, and routine follow-ups for a designated patient panel.
- Develop, implement, and monitor individualized care plans for patients with chronic and complex health issues.
- Collaborate with interdisciplinary team members to optimize patient care and coordinate services.
- Utilize EMR systems (Canopy and Greenway) for accurate documentation and continuity of care.
- Participate in quality improvement initiatives and adhere to value-based care principles.
- Engage patients in preventive health activities and educate them on managing their health conditions.
- Access specialist consultation platforms (e.g., RubiconMD) to enhance diagnostic and treatment decisions.
- Support patient engagement in community room activities, wellness programs, and transportation assistance services.
- Maintain compliance with all regulatory, legal, and ethical standards in clinical practice.

Qualifications & Requirements
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or board eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary state medical license.
- Bilingual proficiency in Spanish preferred.
- No current residents; position is for fully licensed physicians.
- Fellowship training in Geriatrics is preferred but not required.
