Primary Care Physician (MD/DO) - Summary
- Full-time Primary Care Physician (MD/DO) role in an innovative, patient-centered healthcare setting

Duties & Responsibilities
- Deliver comprehensive outpatient primary care to a senior patient population, focusing on those with 3-5 complex diseases such as heart disease, diabetes, and hypertension
- Manage an average daily caseload of 10-12 patients, with a maximum panel size of 650
- Collaborate closely with a dedicated care team including a medical assistant, scheduler, and referral specialist
- Participate in a call schedule shared among local providers (e.g., 1:32 weeks, depending on physician numbers)
- Utilize value-based care strategies to improve patient satisfaction, reduce hospitalizations, and meet HEDIS quality standards
- Perform office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Engage in ongoing professional development and maintain compliance with all regulatory requirements
Salary & Benefits
- Competitive base salary starting at $260,000 with the potential to earn up to $312,000 or more based on performance

Qualifications & Requirements
- Medical degree (MD or DO) from an accredited institution
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM)
- Current, unrestricted, and active license to practice medicine
- Current DEA registration
- Proficiency in office-based procedures such as suturing, joint injections, skin biopsies, and basic wound care
- Fellowship training in Geriatrics is a plus but not required
