Primary Care Physician (PCP) Job OverviewWe are seeking a dedicated and compassionate Primary Care Physician (PCP) to join a dynamic and innovative healthcare provider, offering outpatient care in Greensboro, NC. Responsibilities- Provide comprehensive outpatient primary care services to a panel of up to 650 patients, predominantly seniors managing 3-5 complex chronic diseases such as heart disease, diabetes, and hypertension.
- Deliver patient care in accordance with best practices and company values, ensuring high patient satisfaction and continuity of care.
- Collaborate with a dedicated care team, including medical assistants, schedulers, referral specialists, and support from leadership, to ensure optimal patient outcomes and coordinated care delivery.
- Participate in a value-based care model, focusing on quality metrics such as HEDIS compliance, patient satisfaction, and reduction of hospitalizations.
- Maintain a manageable daily patient load, averaging 10-12 patients per day, allowing for thorough patient interactions and personalized treatment plans.
- Participate in call schedules based on the number of providers in the market, supporting continuity of care outside regular clinic hours as needed.
- Utilize proficient procedural skills in office-based treatments including suturing, joint injections, skin biopsies, and basic wound care.
- Engage patients in wellness programs and activities that promote better health, such as on-site exercise, art classes, and other offerings designed to enhance quality of life.
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree.
- Board Certified or Board Eligible in Family Medicine (FM) or Internal Medicine (IM).
- Fellowship training in Geriatrics is a plus, but not required.
- Current, unrestricted, and active license to practice medicine.
- Current DEA registration.
- Proficiency in common office procedures, including suturing, joint injections, skin biopsies, and basic wound care.
