Primary Care Physician (MD/DO) – Summary
- Deliver comprehensive, value-based primary care to diverse Medicare populations in Chicago, IL.

Duties & Responsibilities
- Provide primary care services to Medicare patients, focusing on preventive care, chronic disease management, and patient education.
- Utilize a value-based care approach to improve patient outcomes and reduce unnecessary hospitalizations.
- Work collaboratively with a multidisciplinary care team (including scribes, medical assistants, and social workers) to coordinate comprehensive patient care.
- Conduct regular patient visits (20-minute standard, 40-minute for new/diagnostic visits), seeing an average of 13-16 patients per day.
- Leverage advanced EMR systems (Canopy and Greenway) for documentation and care coordination.
- Access on-site laboratory services and specialist consults through RubiconMD.
- Participate in community health initiatives and patient engagement activities (including wellness events and educational programs).
- Support and mentor fellow providers and contribute to a positive, inclusive clinic culture.

Qualifications & Requirements
- Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or board eligibility in Internal Medicine or Family Medicine required.
- Active, unrestricted state medical license in good standing.
- Bilingual in Spanish preferred to serve diverse patient populations.
- Fellowship training in Geriatrics is preferred but not required.
- Applicants must not be currently in residency; this role is suited for practicing physicians.
