Primary Care Physician (MD/DO) – Chicago, IL – Full-time
We are seeking a dedicated and compassionate Primary Care Physician (MD/DO) to join our innovative, value-based healthcare network in Chicago, IL. As a full-time Primary Care Physician, you will play a vital role in delivering equitable and effective primary care to the Medicare patient population, focusing on patient outcomes and preventive medicine. This position is ideal for physicians who are committed to high-quality, patient-centered care and are looking to make a lasting impact within the Chicago community.
Responsibilities
- Provide comprehensive primary care services to a diverse Medicare patient population within a collaborative, multidisciplinary team setting.
- Deliver patient care in accordance with value-based care principles, prioritizing preventive medicine and positive health outcomes over volume of services.
- Manage a daily patient panel of approximately 13-16 patients, ensuring adequate time for thorough assessments (20-minute regular visits, 40-minute full diagnostic or new patient visits).
- Work alongside a robust care team including a scribe, medical assistant, and social worker to optimize patient experience and support holistic care delivery.
- Utilize on-site laboratory services, including phlebotomy, and access to specialist consultation through RubiconMD.
- Document patient encounters and coordinate care using electronic medical records (EMR) systems Canopy and Greenway.
- Participate in community-based patient activities, such as exercise programs, events, yoga, dance, and wellness days, to promote patient engagement and well-being.
- Contribute to quality improvement initiatives and support the organization's mission to reduce hospital admissions and enhance preventive care within Chicago, IL.
Qualifications
- Medical Doctor (MD) or Doctor of Osteopathy (DO) degree from an accredited institution.
- Board certification or board eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted, and non-probationary medical license to practice in Illinois.
- Bilingual in Spanish is required to effectively serve the local patient population in Chicago, IL.
- Fellowship training in Geriatrics is preferred, but not required.
- Residents are not eligible for this full-time Primary Care Physician (MD/DO) position.
