Primary Care Physician (MD/DO) - Summary
- Deliver comprehensive primary care to a diverse Medicare patient base in a collaborative and supportive setting.
- Focus on preventive health, chronic disease management, and acute care within a value-based model.
- Work as part of multidisciplinary teams, including scribes, medical assistants, social workers, and other healthcare professionals, to ensure holistic patient care.
- Utilize modern electronic medical records (EMR) systems such as Canopy and Greenway to streamline patient care and documentation.
Duties & Responsibilities
- Provide patient-centered primary care, including diagnosis, treatment, and preventive services to Medicare beneficiaries.
- See an average of 13-16 patients per day, ensuring adequate time for each encounter (20-minute visits for established patients, 40 minutes for diagnostics/new patients).
- Work closely with a dedicated care team, including scribes, medical assistants, and social workers, to optimize patient outcomes.
- Coordinate care and provide referrals using in-clinic resources, including on-site labs with a phlebotomist and access to RubiconMD for specialist consultations.
- Engage patients in community activities within the clinic, such as exercise classes, wellness events, and educational sessions, to promote long-term health and well-being.
- Participate in quality improvement initiatives and adhere to value-based healthcare principles to drive better outcomes and reduce unnecessary hospital admissions.
- Maintain accurate and timely documentation in the EMR system.
- Actively contribute to a supportive team environment, collaborating with other Primary Care Physicians (MD/DO) and Nurse Practitioners within the clinic.

Qualifications & Requirements
- Graduate of an accredited Medical Doctor (MD) or Doctor of Osteopathy (DO) program.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- Active, unrestricted state medical license in good standing.
- Bilingual Spanish language skills required to meet the needs of the community.
- Fellowship training in Geriatrics is preferred but not required.
- No current residents; this position is for experienced physicians only.
