As a Primary Care Physician, you will have the opportunity to work with a supportive, integrated care team including Nurse Practitioners (NPs), Physician Assistants (PAs), Medical Assistants, Social Workers, and access to lab and pharmacy support. Serving a majority Medicare Advantage population, your focus will be on chronic disease management, preventive medicine, and improving the overall health outcomes of your patients. With a patient census of 12 to 15 per day and visit lengths of 20 to 40 minutes, you can deliver thorough, individualized care without the burden of RVU-driven productivity pressure. This approach allows you to make a genuine impact on the lives of seniors while practicing medicine in a way that aligns with your values.

Primary Care Physician (Internal Medicine, Value-Based Care, PCP) - Summary- Deliver value-based primary care to adults aged 65+ (majority Medicare Advantage)
- Focus on chronic disease management, preventive care, and patient wellness
- Work collaboratively within an integrated care team
- Participate in a comprehensive onboarding and training program
- Enjoy a supportive, patient-focused practice environment
- Provide high-quality, comprehensive primary care to an adult patient panel (mostly aged 65+)
- Conduct thorough patient assessments and develop individualized care plans
- Manage chronic conditions and address preventive health needs in accordance with value-based care principles
- Maintain patient census of 12–15 patients per day with 20–40 minute visit lengths
- Collaborate with an integrated care team, including NPs, PAs, Medical Assistants, Social Workers, and other support staff
- Utilize electronic medical records (EMR) efficiently; training provided (Athenacurrently in transition)
- Participate in regular team meetings and contribute to a culture of continuous improvement
- Engage in ongoing professional development, including a 3-week onboarding program focusing on value-based care, documentation best practices, and care model workflows
- Base salary competitive with market rates, commensurate with experience
