Primary Care Physician – Internal Medicine or Family Medicine, VBC – Summary- Deliver high-quality, preventive care for adults aged 65+
- Work within a value-based care model focused on patient outcomes
- Collaborate with a multidisciplinary integrated care team
- Emphasis on meaningful patient relationships and time per visit
- Position based in Nashville, TN with full-time employment
- Provide comprehensive primary care services to a patient panel of adults 65+, with a focus on chronic disease management and preventive health
- Average daily census of 12-15 patients, allowing for 20-40 minute visit times to ensure quality care
- Work collaboratively with nurse practitioners (NPs), physician assistants (PAs), medical assistants, social workers, laboratory, pharmacy, and referral coordinators
- Engage in multidisciplinary care planning to improve health outcomes
- Participate in value-based care initiatives and leverage best practices for documentation and EMR optimization
- Contribute to organizational mission by driving positive impact in senior health within the community
- Participate in a comprehensive onboarding and training program to ensure familiarity with value-based care workflows and integrated care models
- Competitive base salary with first-year total compensation exceeding $300,000, depending on experience and performance
- Commencement and sign-on bonuses available

- MD or DO degree, Board Certified or Board Eligible in Internal Medicine or Family Medicine
- Geriatric care experience preferred
- Experience with HEDIS measures is a plus
