Primary Care Physician (Internal Medicine or Family Medicine, VBC) - Summary

Duties & Responsibilities
- Provide comprehensive primary care to a panel of adults aged 65 and older, focusing on chronic disease management, preventive care, and health maintenance.
- Conduct thorough patient assessments, develop care plans, and manage acute and complex medical issues.
- Work closely with a multidisciplinary care team, including nurse practitioners, physician assistants, medical assistants, social workers, and pharmacy staff, to ensure coordinated, high-quality care.
- Maintain a manageable patient census, typically 12-15 patients per day, allowing for extended visit lengths (20-40 minutes per patient) and a focus on relationship-building.
- Utilize an integrated electronic medical record (EMR) system to document patient encounters, track outcomes, and support care coordination.
- Participate in quality improvement initiatives and performance measurement projects, with an emphasis on meeting HEDIS and other value-based care metrics.
- Engage in ongoing training and professional development, including a comprehensive initial onboarding program.

Qualifications & Requirements
- MD or DO degree with completion of an accredited residency in Internal Medicine or Family Medicine.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Experience with geriatric patient populations is preferred.
- Previous exposure to value-based care models, HEDIS measures, and preventive medicine is a plus; training will be provided for those new to VBC.
- Eligible for and able to obtain an unrestricted medical license in South Carolina.
- Excellent communication skills and a strong commitment to collaboration within a team-based model.
