Primary Care Physician (Internal Medicine or Family Medicine, VBC) – Summary
- Deliver high-quality, preventive primary care to adults 65+ using a value-based care model
- Work collaboratively as part of a multidisciplinary care team

- Full-time position located in Winston-Salem, NC
Duties & Responsibilities
- Provide comprehensive primary care for adults aged 65+, with a focus on chronic disease management and preventive health
- Conduct thorough patient assessments, develop individualized care plans, and monitor ongoing health status
- Maintain a schedule of 12–15 patients per day, allowing for in-depth patient engagement and quality care
- Participate in a multidisciplinary team including nurse practitioners (NPs), physician assistants (PAs), medical assistants, social workers, lab and pharmacy support, and referral coordinators
- Coordinate care transitions, referrals, and follow-up for optimal patient outcomes
- Utilize electronic medical records (EMR) efficiently, with ongoing support and training
- Engage in ongoing training, including a comprehensive onboarding program focused on value-based care, documentation, and integrated care models
- Support and mentor team members as needed

Qualifications & Requirements
- Board certified or board eligible in Internal Medicine or Family Medicine
- Geriatric experience preferred, but not mandatory
- Experience in value-based care or HEDIS measures is a plus
- Eligible for medical licensure in North Carolina
- Commitment to evidence-based, patient-centered care
- Strong communication, organizational, and teamwork skills
